Women must be pregnant or up to 24 months post-partum, used alcohol or drugs during pregnancy and are ineffectively connected to community services

Description
This program offers 3 years of home visits and case-management for pregnant and parenting women with substance use issues. The goals are to help moms build healthy families and prevent substance use with future pregnancies.
