Eligibility

Individuals enrolled in a Federally Recognized Tribe; individuals who are first-generation and second-generation descendants of those Tribal members; Self-attesting Natives; Individuals with Medicaid who are members of a Native Household; Employees of Cowlitz Indian Tribe and their family members

Required documents

Photo Identification Tribal Enrollment, Certificate of Indian Blood, or Letter of Descendancy from Tribe Birth Certificate(s) (if registering as a descendant) Marriage/Divorce Papers to Reflect Name Changes from Birth Certificate (if necessary) Current Insurance Information (Medicaid, Medicare, or Private Insurance). If you do not have insurance, we will support you by helping you apply for alternative resources.

Provides counseling and behavioral health services for individuals and families. Incorporate holistic and cultural practices. Serves tribal members only.

Cowlitz Indian Tribe

Offers primary and preventative medical care, chronic disease management, family planning, behavioral health, dental, WIC, maternal support services/infant case management, and substance use disorder treatment in both residential and outpatient settings.
