Child/Youth aged 6-22 Live in Bellevue, Kirkland, Redmond or Sammamish

Required documents

Insurance card or medical coupon and Photo ID Pay stub or proof of income if needing help to apply for reduced fees Referral paperwork from a school, physician or law enforcement (if applicable)

Description

Provides age-appropriate and culturally sensitive behavioral health services to children, youth and their families. Offers specialized programs for ethnic and sexual minorities; youth with disabilities; and youth involved with the juvenile justice system. Accepts Apple Health (Medicaid) and offers a sliding fee scale. Serves children and youth who live in Bellevue, Kirkland, Redmond, and Sammamish.
