Eligibility

Any enrolled American Indian/Alaska Native (AI/AN) of a federally recognized tribe, any lineal descendant of a federally recognized AI/AN (grandparent and/or parent MUST be enrolled), dependents or spouses of an enrolled Cowlitz tribal member with billable insurance, a tribal employee, their dependents, and/or their spouse with billable insurance.

Description

Provides a range of wellness services to our patients and community. With focus on promoting well-being through prevention, treatment, and education. Primary care services include child well checks, preventative care, family planning, women's health, and immunizations.
