The Apple Health Core Connections (AHCC) program is a managed care program that offers coordinated health care services to children and youth in foster care, adoption support, guardianship assistance program, extended foster care, Unaccompanied Refugee Minors, young adults age 18-26 who aged out of foster care on or after their 18th birthday, and children and youth who reunified with their parents (up to 12 months after foster care ends).
