Children and Youth with Special Health Care Needs (Jefferson County) at Jefferson County Public Health (Port Townsend)
Children and Youth with Special Health Care Needs (CYSHCN) program serves children 0-18, who have or are at risk of developing, serious physical, developmental, behavioral, or emotional conditions. Program staff helps connect them and their families to the health and related services they need to thrive. This service also provides diagnostic services for children with Autism. The SMART Team assists local families with timely access to Autism evaluation and referrals to local supports and services. Anyone can make a referral for a child including medical professionals, school districts, service providers, and parents.
