The following information will be required: Provider One number, name of medical provider, name, address and phone number of medical facility, appointment date and time, and reason for the appointment. Also offers eligible clients gas cards and reimbursement options for parking, bridge toll and ferry fees. Must be enrolled in Apple Health (Medicaid).
