Pregnant people and children (under age 5) in all kinds of families who meet income criteria

Required documents

Proof of ID, proof of income for the family, and proof of residence in Washington State.

Mon - Fri 9:00am - 4:30pm until 7pm on the 2nd & 3rd Wednesdays Sat 9:00am - 1:00pm

Application process

Call to make an appointment. Remote appointment options available Monday to Friday 9am to 4:30pm, until 7pm on the 2nd & 3rd Wednesdays, and from 9am – 1 pm on the third Saturday.
