The Apple Health Foster Care (AHFC) program is a managed care program that offers coordinated health care services to children and youth in foster care, children and youth in adoption support, extended foster care for 18-21 years of age, and 18-26 years of age who aged out of foster care on or after their 18th birthday. Children and youth reunified with their parents (eligible for 12 months after foster care ends)
